Survey Details

  • Time Commitment: Approximately 25 minutes.
  • Structure: 48 thoughtful, multiple-choice and open-ended questions.
  • Flexibility: You can skip any question, return to edit previous responses, or pick up where you left off at any time.
  • Privacy: All responses are confidential. Please refrain from sharing specific personal information about your clients.
  • Incentive: As a token of our appreciation, three participants will be randomly selected to receive a copy of our new book, The Chronic Disorganization Tapestry. (Entry requires providing your name and email address).
